Insurance Providers
Insurance companies issue coverage. Licensed producers, including insurance agents and brokers, arrange or service it. Choose the path that matches the applicant’s actual role. An insurance broker here means a broker of insurance, not a freight broker.
These criteria guide evidence review for this role alongside the common Dalilah Standard requirements. Submission does not grant recognition.
Requirements for recognition review
Legal identity and authority
Identify the applying legal entity, its owners and authorized representative. Insurers document the states and lines in which they may issue coverage. Producers, insurance brokers, and agents document their individual and business entity licenses, lines of authority, and appointments or insurer authorizations where applicable.
Transportation exposure and truthful submissions
Describe the transportation business, operations, vehicles, drivers, cargo, and other material exposure information relevant to the coverage offered or placed. Document how inconsistencies and material changes are investigated and corrected before relying on them.
Coverage accuracy
Identify the issuing insurer, policyholder, coverage type, policy period, limits, and material exclusions or conditions in communications that describe coverage. Clearly distinguish a quote, binder, certificate, policy, and requested change; do not imply coverage has begun before it has.
Changes, cancellation and fraud response
Maintain a process for changes, renewal, cancellation, nonrenewal, complaints, and suspected misrepresentation. Give required notices to the proper parties, preserve the underlying record, and avoid presenting a certificate or data feed as conclusive evidence of current coverage.
Privacy, records and conflicts
Limit access to customer and claim information, retain records of material representations and decisions, and disclose material compensation, appointments, ownership, or other relationships that could influence recommendations or program review.
Role limits and continuing review
An insurer remains responsible for its own underwriting and claims decisions. A producer, broker, or agent must not imply that a Dalilah pledge or recognition guarantees coverage, insurability, claim payment, or an insurer endorsement. Update the Council when licenses, appointments, authority, or material practices change.
Evidence to prepare
Insurers should prepare company identity and state authority evidence, transportation coverage descriptions, sample policy or certificate controls, and change/cancellation procedures. Producers, brokers, and agents should prepare applicable individual and business entity license details, insurer appointment or authorization evidence where applicable, a sample coverage explanation and correction workflow, and relevant conflict disclosures. Redact customer and claim information before sharing evidence through the controlled review process.
Do not upload sensitive customer, driver, government ID, or shipment records in the initial application. The review team will request evidence through a controlled process.
